OHSAA Physical Form

Please download and fill this form out online.  Then print it out to take to your child’s annual physical appointment for a physician’s signature.  When all pages are filled out and signed by you, your child, and a physician, please upload them to DragonflyMAX or turn them into the Athletic Department.

*Note for parents/guardians and athletes: You may type your name in the appropriate signature lines before printing, and that will serve as your digital signature.
